This agreement is for the total amount shown above between the Customer named in this form (“Customer”) and Set the Table for Me, LLC (“Set the Table for Me”, collectively “Parties”) for the rental or purchase of the items and services described as listed in this form. Parties agree to be bound by the terms and conditions contained in this Agreement for the materials, goods, and/or products set forth in the attached form and described herein (collectively, the "Goods") rented or sold to Customer named in the attached form during the rental period requested which will be delivered and picked up at the address requested in this form.
